Are you a referrer in Cambridgeshire or Peterborough?

As a GP or health professional, you will already be aware that the risk of Type 2 diabetes is significantly reduced if your patients make positive changes to their diet, weight and the amount of physical activity they do.

Your local Healthier You service can support your patients in taking action in all of these areas. Taking this kind of action now is very important as it can reduce your patients’ risk of, or even stop the development of, the very serious health condition of Type 2 diabetes.

Who's Eligible

Your patient is eligible if they are motivated to take part in light exercise and meets the following criteria:

  • HbA1c results between 42–47mmol/mol (6.0– 6.4%) in the last 12 months or
  • Fasting plasma glucose result between 5.5-6.9 mmols/l in the last 12 months
  • Aged 18 years or over
  • Registered with a GP Practice within Cambridgeshire and Peterborough
  • Not pregnant
  • Does not have Type 2 Diabetes
  • There is no medical reason why this patient should not take part in a programme that includes light-moderate physical activity

Identifying and referring patients

  • Clinical systems will alert where patients may be eligible for a referral to ICS
  • You should offer patients a referral to ICS using the ICS referral form on your clinical system
  • The offer of a referral would include the opportunity to have a “motivational” conversation with ICS
  • If you identify patients who meet the eligibility criteria during NHS Health Check, please refer them to ICS (instead of other local lifestyle services e.g. Everyone Health) using the form on your clinical system
  • Referrals should be sent via nhs.net mail – the address is on the ICS referral form

Retrospective searches

In order to identify any patients who have had a blood test in the last 12 months and could be eligible for the programme, we have put in place the following arrangements to support practices in inviting patients to participate in the programme.

  • The CCG Primary Care Information Team has developed a “search” for you to identify patients eligible for referral to the programme
  • Once you have signed a data processing agreement with ICS, your practice will then send ICS the patient details
  • ICS will send the referral invitation to those patients on behalf of your practice (using your headed paper)
  • The offer of a referral would include the opportunity to have a “motivational” conversation with ICS

Resources to support you

  • The GP information pack here contains further details on the programme. A hard copy has been sent to your practice also
  • The data processing agreement (with reference to invitations sent to patients identified through the retrospective searches) will be sent to you by the CCG by the end of October
  • A ‘contact us’ section specifically for healthcare professionals will also be available on the Healthier You site. If however, your enquiry is urgent you can talk to your ICS local Service Delivery Manager, Jan Gould Martin on 077425622108.

Feedback on your patient’s outcomes

  • Feedback will be emailed to you via nhs.net about your patient’s outcomes following their participation in the programme

How to refer

To refer an individual to the Healthier You Programme, please ensure they meet the eligibility criteria then complete the referral form which is embedded into your clinical system and send it to:
