You’ve been referred to the NHS Healthier You: Diabetes Prevention Programme. The first step in the programme is to complete an initial assessment over the phone and then join one of our group sessions.
Initial phone assessment
The initial phone assessment will take around 40 minutes to complete and you can tell us which day(s) of the week would be best to call you on.
Remote group sessions
You will learn how to prevent Type 2 diabetes, have the opportunity to partake in group discussions and ask your coach questions, all from the comfort of your home.
These sessions are usually delivered face-to-face, however, due to the Coronavirus, our sessions are being delivered remotely for the foreseeable future. As an alternative, we are offering two options for remote delivery to ensure the sessions are accessible to all:
- Interactive online sessions hosted via Microsoft Teams
- Telephone sessions

Once you’ve found a suitable group in the table below, use the form underneath to request to join and to tell us your preferred day(s) for the initial phone assessment. We’ll email you with confirmation of your booking shortly afterwards.
Please note: Spaces for each session are limited and are allocated on a first-come, first-serve basis. We recommend requesting your place in the group as soon as possible.

The NHS Healthier You: Diabetes Prevention Programme is designed to help you take control of your health, supporting you to make changes to your diet, weight and the amount of physical activity you do. Taking action now is very important as it can stop you developing Type 2 Diabetes.
