What the Healthier You programme offers

We provide tailored, personalised support to reduce your risk of type 2 diabetes. You’ll have the chance to learn about healthy eating, losing weight and exercising. This combination of lifestyle changes is proven to help prevent the disease.

Would you like to hear more about the NHS Diabetes Prevention Programme and how we could support you in reducing your risk of developing Type 2 diabetes? Then why not sign up to one of our upcoming Patient Information Sessions. This programme is a free service for people living with prediabetes and those who have a history of gestational diabetes. The programme combines specialist nutrition, psychology and physical activity to promote long term behaviour change.

You are eligible for the programme if you’re:

  • Aged over 18
  • Registered with a GP in one of our locations
  • Not pregnant (unless you currently have Gestational Diabetes)
  • Able to do light/moderate physical activity 
  • You have not been previously diagnosed with type 2 diabetes  

If you are at risk of diabetes and eligible for the programme, you can join by getting a referral from your GP, nurse or health care professional.

What to expect on the In Person Group programme

Initial one-to-one assessment

This is a chance to talk about your health and motivation as you embark on your journey to reduce your risk of developing type 2 diabetes.​Should further support be required with your motivation and goals, coaching calls can also be offered to support your experience.​

Find local venues

Guidance and support during sessions

You will take part in 6 fortnightly and 7 monthly group sessions at a local venue. You’ll learn about nutrition, physical activity and strategies for long-term progress, connecting with others who are on the same journey.   

End of programme appointment

Your qualified health coach will recap what you’ve learnt and help you set future goals and any extended support you may need. This is also the chance to celebrate your achievements!  

Continued access to the Wellbeing Way app

After you finish the programme, you’ll have continued access to the Wellbeing Way app for a further 12 months.  

What to expect on the digital programme

To access full functionality from the digital service, a device compatible with IOS 17 is required.

A personalised health care plan

When you join, we will assess your needs and preferences to create a personalised health care plan.

Choose your journey

You can opt for a personal health coach or a coach-led support group. You can also choose to receive support via the Oviva app or via phone/video calls. 

Track your progress

The app helps you to track your food, mood and activity. It’s an easy way to keep in contact with your health coach. During the programme, you’ll also have access to learning modules. 

Lifetime access to the app

After your programme ends, you’ll have lifetime access to the app and learning modules.

What to expect on the Tailored Remote Programme

Here’s what takes place during the Tailored Remote Programme, for people who: need BSL; have a visual impairment; need language support; or are women with previous gestational diabetes. Sessions will be hosted on Microsoft Teams with BSL interpreters present.

Tailored to specific groups

All session content is tailored to each group and delivered by a qualified health coach. 

Remote sessions

Attend your session from the location of your choice via Microsoft Teams alongside individuals with shared experiences.

Continued access to the Wellbeing Way app

After you finish the programme, you’ll have continued access to the Wellbeing Way app for a further 12 months.
