Living with Type 2 diabetes can affect every part of daily life, especially when mobility, confidence or routine are disrupted. The NHS Type 2 Diabetes Path to Remission (T2DR) Programme, delivered by Xyla, supports people to take meaningful steps towards better health through structured nutrition, personalised coaching and a clear pathway to long-term lifestyle change.
Designed for people who want to improve their blood sugar levels and work towards remission, the programme offers a clinically supported approach that helps people build new habits that last.
The programme guides participants through three key phases:
Throughout, participants receive personalised coaching and access to tools that help them stay motivated and track their progress.
When Steven joined the T2DR Programme in April 2024, he was navigating a particularly challenging period of life. After spinal surgery in 2022, his mobility reduced significantly and his weight increased. Reflecting on that time, he says:
“I didn’t think my diet was terrible, but it included weekend takeaways and far too much processed food. Looking back, the symptoms of Type 2 diabetes were creeping up on me; constant tiredness, excessive thirst and frequent trips to the bathroom day and night.”
His diagnosis motivated him to take action. After hearing about T2DR from his GP, Steven enrolled with the hope of reclaiming his health, energy and independence.
Total Diet Replacement was challenging at first but Steven soon began to see the benefits. He found the educational elements of the programme particularly valuable, helping him cut through confusing diabetes messaging. Xyla’s Wellbeing Way app became central to his progress, supported throughout by his coach.
“The support from my practitioner was excellent. Her information sharing and pep talks kept me on track.”
Family, colleagues and friends played a big part too, giving him space to focus on his goals and understand that this was a shortterm commitment for longterm gain. Navigating birthdays and social events became easier as he took on the role of designated driver. As an artist and illustrator, weight loss brought unexpected benefits:
“Since losing weight, I can now walk to my studio and back rather than drive. This newfound mobility has been incredibly liberating.”
With the support of the T2DR programme, Steven achieved significant improvements in his health and wellbeing:
His creativity has flourished too: a recent piece was highly commended in a national competition and he continues to share his work online.
“My overall health is the best it has been in as long as I can remember…My advice to any new user is to stick with it. It’s tough and not for the fainthearted but learn as much as possible and keep going. You’ll get there.”
Steven’s story illustrates the profound impact specialist diabetes remission programmes can have:
These changes not only improve individual quality of life but also reduce pressure on wider healthcare services.
The T2DR Programme supports people to take control of their health, reduce their risk of complications and, in many cases, work towards diabetes remission. By empowering individuals with the knowledge and tools to make sustained changes, we can help communities live longer, healthier lives.
