Register for the NHS Diabetes Prevention Programme

Can you make a self-referral? 

To take part in the Healthier You: NHS Diabetes Prevention Programme, you need to be referred on to this programme by your GP. 

If your GP has sent you a letter or text that confirms you are at risk of type 2 diabetes, you can register online using the form below. 

To register, all you need is the following information, which is detailed in your letter: 

  • Your NHS number  
  • Your latest HbA1c or FPG reading, including the date  
  • Your GP surgery name 

If you meet any of the following, please speak to your GP, as you are unfortunately ineligible for the programme: 

  • You have previously been diagnosed with type 2 diabetes 
  • You are currently pregnant 
  • You have an active eating disorder 
  • You consider yourself to have severe/moderate frailty 
  • You have undergone bariatric surgery in the last 2 years 

If you have a history of Gestational Diabetes (GDM) and have received a letter from your GP about our service, please do not complete the below form. Please call us on 0333 577 3010 or email info@preventing-diabetes.co.uk. Our Patient Support Team can register you instead.  

Please do not complete this form if you have already registered with Xyla Health & Wellbeing – registering a second time could delay your referral. 

If you have been issued with a Service User ID (e.g. su4fum), this confirms your registration on to the diabetes prevention programme and you do not need to complete this form.
