We are excited to welcome you to the Healthier You: Diabetes Prevention Programme.
This free 9-month programme provides tailored, personalised support to reduce your risk of developing type 2 diabetes.
You’ll have the chance to learn about healthy eating, losing weight and exercising. In turn, this will help you to make positive lifestyle changes and reduce your blood sugar levels.
The group-based programme has four stages:
Initial assessment: A one-to-one phone appointment to talk about your current health and lifestyle. This can take place during your initial phone call with us, when making your booking, or we can pre-book for a time/day more convenient to you.
6 fortnightly group sessions: These focus on your personal diabetes risk and the impact diet and physical activity has on this. We will also explore how different areas of wellbeing, such as sleep and stress, play an important role in your health.
7 monthly group sessions: Building on the knowledge you have gained so far, you will learn new skills and strategies to help you maintain your lifestyle changes long term.
End of programme review: A group session to celebrate your achievements and find out how to sustain the lifestyle changes you have made on the programme.
Outcomes:
70% of service users on the group-based programme reduced their HbA1c to a healthy range. 80% of those who start the programme overweight reduce their weight.
The digital programme has 3 stages:
Start: You’ll tell us a little more about yourself, your health and wellbeing, and you can choose to be matched with your own personal coach or coach-led support group.
You will have a first consultation with a health coach who will help you prepare for the journey ahead. They will create an action plan to help you achieve your goals over 9 months.
Change: The first 12 weeks of the programme focuses on behaviour change to work towards your action plan.
You will have support from your health coach or coach-led peer support group. This will help you in this early stage of making long-term changes to your lifestyle.
Sustain: For the remainder of the programme, you will continue to build confidence in maintaining your new, healthier behaviour long-term, helped with monthly support.
You will continue to self-track your progress on the Oviva app and grow your knowledge using the weekly online resources, even after the programme ends.
