The NHS Healthier you: Diabetes Prevention Programme is held in a number of community venues across Somerset.
If we don’t have a suitable group available for you to book, we’re here to help you take your next steps to a healthier you.

3) We will then contact you once a group becomes available. We will contact you through either SMS, email, phone or letter.
Join our waiting list- If no venue fits your needs, you might consider joining our Digital Programme or one of our remote Group Sessions via Microsoft Teams or via our Tailored-remote service. You can also click here to suggest a venue for our team to review.

What should I do if none of the venues are suitable for me?
If none of the venues listed are suitable for you, we recommend that you choose our Digital Programme or our Tailored Remote Service (TRS) where you can access groups remotely via Mircosoft Teams (suitable for people who have a hearing or visual need, need a language specific group (Bengali, Urdu, Punjabi, Gujarati) or with a previous history of gestational diabetes). Alternatively you can request to be discharged back into the care of your GP for additional support.
Digital programme
Participate in our program conveniently through an app, delivered in partnership with Oviva. To get started, simply click the button below and opt in for the digital program. To access full functionality from the digital service, a device compatible with IOS 17 is required.
Tailored-remote service
Access groups remotely via Mircosoft Teams (suitable for people who have a hearing or visual need, need a language specific group (Bengali, Urdu, Punjabi, Gujarati) or with a previous history of gestational diabetes.
Discharge from the programme
Please take a moment to complete our short form. Once you have filled it out, we will proceed to discharge you back into the care of your GP. This will ensure that you continue to receive follow-up care and support.
