Welcome to the NHS Diabetes Prevention Programme

We are excited to welcome you to the Healthier You: Diabetes Prevention Programme.

This free 9-month programme provides tailored, personalised support to reduce your risk of developing type 2 diabetes.

You’ll have the chance to learn about healthy eating, losing weight and exercising. In turn, this will help you to make positive lifestyle changes and reduce your blood sugar levels.

There are 3 ways to take part in the Healthier You: Diabetes Prevention Programme: 

Group-based sessions

  • Our interactive sessions are led by qualified coaches who will help you understand and establish an effective strategy to reduce your risk of developing type 2 diabetes 
  • Social support is a key aspect of long-term success – through these group sessions you will connect with others who are on the same journey  
  • Sessions are fully supported with our bespoke Wellbeing Way app, online learning and our community Facebook group  
  • With over 200 new groups starting every month, we are sure there will be a group to suit you. Use our Guest Access to find your nearest groups. You must log in before you can book.

The group-based programme has four stages: 

Initial assessment: A one-to-one phone appointment to talk about your current health and lifestyle. This can take place during your initial phone call with us, when making your booking, or we can pre-book for a time/day more convenient to you. 

6 fortnightly group sessions: These focus on your personal diabetes risk and the impact diet and physical activity has on this. We will also explore how different areas of wellbeing, such as sleep and stress, play an important role in your health. 

7 monthly group sessions: Building on the knowledge you have gained so far, you will learn new skills and strategies to help you maintain your lifestyle changes long term. 

End of programme review: A group session to celebrate your achievements and find out how to sustain the lifestyle changes you have made on the programme.

Outcomes:

70% of service users on the group-based programme reduced their HbA1c to a healthy range. 80% of those who start the programme overweight reduce their weight. 

Digital one-to-one or peer support

  • Specialist and personalised dietary support from a health coach over 9 months. This can be one-to-one or part of a peer support group. It’s available on your smartphone with Oviva’s NHS Digital-approved app.   
  • Entirely remote with no need to travel, so you can access support wherever you are.  
  • Life-time access to psychological and nutritional advice on the app, to help you gain confidence and keep up positive lifestyle changes long term. 

The digital programme has 3 stages:  

Start: You’ll tell us a little more about yourself, your health and wellbeing, and you can choose to be matched with your own personal coach or coach-led support group.

You will have a first consultation with a health coach who will help you prepare for the journey ahead. They will create an action plan to help you achieve your goals over 9 months.  

Change:  The first 12 weeks of the programme focuses on behaviour change to work towards your action plan.

You will have support from your health coach or coach-led peer support group. This will help you in this early stage of making long-term changes to your lifestyle.  

Sustain: For the remainder of the programme, you will continue to build confidence in maintaining your new, healthier behaviour long-term, helped with monthly support.

You will continue to self-track your progress on the Oviva app and grow your knowledge using the weekly online resources, even after the programme ends. 

The Oviva app is available for android and apple devices. To access full functionality from the digital service via iphone, a device compatible with ios 17 is required. 

 

Tailored Remote Service

  • We offer a range of tailored groups, including for people who: require a British Sign Language interpreter; have a visual or hearing need; need language support; or for women with a previous history of gestational diabetes
  • All sessions are run remotely over Microsoft Teams, so you can attend from the comfort of your own home or a location of your choice
  • The sessions are led by qualified coaches who will help you understand and establish an effective strategy to reduce your risk of developing type 2 diabetes. 
  • You’ll learn about nutrition, physical activities and strategies for long-term progress.   
  • Our coaches are on hand to support you throughout your journey and help with any difficulties you may face.  
  • Sessions are fully supported with our bespoke Wellbeing Way app, online learning and our community Facebook group.  
  • Follows the same 4 stages as our in-person sessions
  • Our friendly team can support you in accessing the sessions through our Microsoft Teams guidance
